How do I fill out a W-9 in accordance with the Local Equity Program

A fillable W-9 Form is available on the IRS website. This form is necessary to release any funds awarded. Please note that checks will be issued to the name and address on the submitted W-9. For instruction on how to fill out the applicant's W-9 in accordance with the LEP, please see below. 

  1. If the payment will be issued to the applicant;
    1. Box 1 – Enter legal name
    2. Box 2 – Leave blank
    3. Box 3 – Check “individual/sole proprietor or single-member LLC”
    4. Enter residential address where checks should be received
    5. Part 1 – Enter Social Security Number
    6. Sign and Date
  2. If the payment will be issued to the applicant’s business;
    1. Box 1 – Enter business name
    2. Box 2 – Leave blank
    3. Box 3 – Check appropriate business type
    4. Enter business address where checks should be received
    5. Part 1 – Enter Employer Identification Number
    6. Sign and Date
